The dental SEO market, measured
Fifteen quality agencies compete for “dental seo agency” in the United States; there are others, and this table covers the ones the live SERP put in front of a buyer. We measured the fourteen service pages in that SERP plus our own, against one 152-term list built from what the competitors write. Why contextual density is the yardstick
Across the markets we have measured, the pages that rank carry more of the vocabulary a buyer uses than the pages that don’t, and they carry it as a wide set of distinct terms rather than one phrase repeated. Contextual density is that measurement: the share of a page’s words that belong to the market’s term list, read together with how many distinct terms the page covers and how often each one returns. A high reading means the page speaks the market’s language across the whole topic, which is the clearest on-page sign that an agency understands strong SEO. That is why we measure other SEO agencies’ pages on it, and why it carries most of the weight below.

How to choose a dental SEO company
Start with where the patients actually come from. A dental practice draws from a few miles, so the Google Business Profile and the local pack decide more revenue than the blue links under them. That makes an agency’s own local work fair to judge, and it makes one question worth asking early: how many practices do they already run in your city, and will yours compete with them. Four of the fifteen here state a one-practice-per-market rule; the rest do not say.
Separate the marketing company from the software company. RevenueWell, Adit and PBHS sell patient-engagement or practice-management software with marketing attached, and that can be the right buy if you want one vendor for the phone system, the forms and the SEO. It is a different purchase from Firegang, Delmain, Practice Cafe or Gargle, who sell the marketing and nothing else. Neither is better; they fail differently, and the software vendors’ SEO is usually the part with the least attention on it.
Read their page for your keyword before you read their proposal. The coverage column above is what that reading looks like when a script does it: how much of the vocabulary your patients use appears on the page the agency wrote to win your business. PBHS covers 13% of it and has been in dentistry since 1977. Denteel covers 14% and holds position one. A thin page is not proof of a thin agency, but it tells you how much of the market’s language the agency has written down, which is the thing you are paying them to do for you.
Ask what happens to the parts of dentistry that are not SEO. HIPAA touches call tracking, form submissions and analytics. Reviews decide case acceptance more than position does, and a practice at 4.2 stars loses patients to one at 4.8 while outranking it. Insurance verification and treatment financing decide which enquiries convert. An agency that has only talked about rankings by the end of the first call has not run a dental account for long.
Weigh the domain and the page separately. Thrive has 125,629 US organic keywords and a dentist page covering 36% of this market’s vocabulary; Firegang has 627 keywords and covers 70%. One is a large generalist that wrote a small dental page; the other is a dental specialist that wrote a thorough one. Which you want depends on whether you are buying reach or attention, and for a single-location practice attention usually wins.
Questions to ask before hiring
How many dental practices do you already run in my city, and will mine compete with them? Four agencies here state a one-practice-per-market rule. If an agency will not answer this plainly, assume the answer is more than one.
Show me the page you would write for my highest-value treatment. Implants, Invisalign and full-arch cases pay for the retainer. A page for those has to answer cost, financing and recovery, not repeat the treatment name. Ask to see one they have already published.
How do you handle HIPAA in call tracking, forms and analytics? Every one of those touches protected health information. An agency that has not configured them is exposing the practice, and the answer takes about thirty seconds if they have done it before.
What do you do about reviews, and who asks for them? Reviews convert what rankings deliver. If the answer is a software licence and nothing about the hygiene recall workflow, the reviews will not arrive steadily.
How will you measure the work, and can I recompute it? This page exists because most agency rankings cannot be checked. Ask the same standard of the agency: a metric, a method, and a number you can reproduce next month.
What is the minimum term, and what happens to the content if I leave? Several agencies here publish month-to-month terms. Content written for your practice should be yours; ask for that in writing before the first invoice.
Reading the table
Five pillars make the score, and four of them measure the page. Coverage counts how many of the 152 market terms the page uses. Density passes the page’s contextual density through a saturation curve, so a page at this market’s median of 8.6% scores 46 and a page at 28% scores 74 rather than 100. Efficiency reads the repetition index and gives full marks at 3 or below, where a page’s density comes from breadth rather than from returning to the same terms. Structure reads how much of that vocabulary reaches a title or a heading. Footprint is the one pillar about the firm: branded search volume in the market’s country and Google reviews, on fixed log scales.
Three things about this instrument are said plainly. First, ProStar SEO publishes the table and writes its page against the list the table scores. That is why the list is built only from terms found on at least two competitor pages or in Semrush’s variation data for the anchor: the publisher cannot add vocabulary nobody else uses and then score itself on it. Even so, our page now covers all 152 terms, and a perfect coverage score on your own index deserves the scepticism it invites. Read it as what it is: a page written deliberately against a published list, by the people who published the list. Second, no score depends on any other row, so a competitor rewriting its page cannot move ours and ours cannot move theirs. Third, the publisher’s profile below follows the same template at the same length as the other fourteen.
Position and score disagree in both directions, and the table prints both. Denteel ranks first on Google with 14% coverage on a 674-word page. Our own page covers the whole list and does not appear in the first fifty results for the anchor. Neither is a fault in the instrument. The index measures the page; Google ranks the domain, and on this keyword the domains ahead of us are older and better known than ours.
