We read forty-eight results for “healthcare seo agency” in the United States on 2026-09-18. Thirteen of those results were ranking listicles, four were directories, and three were forum threads. Fifteen quality agencies compete for “healthcare seo agency” in the United States; others exist, and this table covers the ones the live SERP put in front of a buyer on 2026-09-18. Reaching fourteen agency service pages required reading to position 28, because the first forty-eight results held thirteen ranking posts, four directories and three forum threads. We measured those fourteen pages plus our own against one 119-term list built from what the competitors write.

Why contextual density is the yardstick
Across the markets we have measured, the pages that rank carry more of the vocabulary a buyer uses than the pages that don’t, and they carry it as a wide set of distinct terms rather than one phrase repeated. Contextual density is that measurement: the share of a page’s words that belong to the market’s term list, read together with how many distinct terms the page covers and how often each one returns. A high reading means the page speaks the market’s language across the whole topic, which is what a search engine and a language model both have to work from.

How to choose a healthcare SEO company
Start from the constraint that separates this vertical from every other one. Google classifies medical content as YMYL and applies E-E-A-T to it with unusual severity, so a healthcare page has to clear a trust threshold before relevance is even assessed. An agency that cannot say who writes your clinical copy and who reviews it is proposing to spend your budget on pages that will not qualify.
Ask what happens to patient data. HIPAA governs how a site collects and transmits patient information, which turns analytics configuration, form handling and call tracking into an SEO problem as much as a legal one. Three agencies in this cohort publish nothing about compliance on the page a buyer lands on. That absence is not proof of a problem, and it is a question worth asking on the first call.
Separate the specialists from the generalists, then decide which you want. Cardinal Digital Marketing, Target Patients MD, Varn Health, Wisevu and Glacial Multimedia work in healthcare only. Thrive, OuterBox, Coalition Technologies, SeoProfy, Panem and RevenueZen run healthcare as one vertical among many. A specialist knows your regulatory ground; a generalist brings scale and cross-vertical pattern. Neither answer is wrong, and the page you are reading measures only the page, not the engagement.
Read the pricing that is published, and notice who publishes none. Five agencies in this cohort state a figure: uSERP from $10,000 a month, SeoProfy from $3,500, RevenueZen from $3,000, ProStar SEO from $3,000 for professional and healthcare practices, and Medicotech from $1,500. Nine publish nothing. A published floor tells a buyer whether a first call is worth booking.

Questions to ask before hiring
Which of your current healthcare clients can I speak to, and in my specialty? Named clients on a page are a claim; a reference call is evidence. Three agencies in this cohort name no clients at all on the pages we read.
Who writes our clinical content, and who reviews it before it publishes? Google grades healthcare pages on author expertise and review, so the answer decides whether the content can rank. A named medical reviewer with visible credentials is the standard the framework rewards.
How will you keep our analytics, forms and call tracking compliant with HIPAA? Protected health information travelling through an untagged measurement stream is a liability with a ranking cost attached. The agency should be able to name the configuration it uses before you sign.
What does your reporting show beyond keyword rankings and organic traffic? Rankings move first and pay for nothing by themselves. Ask to see booked appointments, patient calls and cost per acquisition, and ask which market the position tracking runs from.
Is this a healthcare programme or a general programme with a healthcare page? Both models work. The buyer should know which one is being sold, and the characteristics table below shows how far each agency’s healthcare work runs beyond the single page we measured.
What are the contract length and the notice period? A twelve-month lock-in protects the agency through the months when nothing moves. A month-to-month term with notice puts the risk on the party doing the work.
Reading the table
Five pillars make the score, and four of them measure the page. Coverage counts how many of the 119 market terms the page uses. Density passes the page’s contextual density through a saturation curve, so a page at the cohort median scores near the middle of the scale rather than at the top. Efficiency reads the repetition index and gives full marks at 3 or below, where a page’s density comes from breadth rather than from returning to the same terms. Structure reads how many of the terms the page covers reach a title tag, an H1, an H2 or an H3. Footprint is the one pillar that is not the page: it averages United States brand search volume against Google review standing.
One disclosure belongs here rather than in a footnote. ProStar SEO’s measured page covers 103 of the 119 terms, which is 86.6% of the list, and our page therefore holds the coverage maximum that every other row is normalised against. That compresses each competitor’s coverage pillar against our own page: Medicotech’s 60 terms read as 50.4 on the scale, where they would read as 100 if the scale ran to the best competitor instead. The normalisation is a frozen parameter of index 4.0 and we have not changed it mid-edition, so the honest thing is to say what it does. A reader who wants the competitor-normalised view can divide any coverage figure by 0.866. We also note the cause: our healthcare page was rewritten against this market’s vocabulary on the edition date, which is exactly what the index rewards and exactly why our own figure should be read with that in mind.
The instrument itself is in better health than the coverage figure suggests. Of the 119 terms, 108 are carried by some competitors and not others, and only one term appears on every competitor page, so the list separates the cohort rather than handing every row the same points.
